Breach of contract in reference to what? There are a few different forms - from a legal perspective

In reference to an extended auto warranty. Warranty was sold 1st claim made and paid 2nd claim made they stated they had sold the wrong policy and the person was not eligible for that policy. 2nd claim was not paid. Policy canceled. 11 payments was made and acepted on the 12 payment installments.

Frances, A more appropriate avenue for assistnace would be through your personal attorney or, if you don't have one, a local legal aid clinic. There really is not a standardized form per se for something like this. The realitiy is that you will probably not receive any response on your own and it may take a letter from an attorney for the insurance company to respond.

I think you are out of luck. This is just my experience, but demand letters are almost worseless. If someone (or some company) hasn't paid, a $1.39 Certified Enevelope with a Letter inside isn't going to help. About the only good a demand letter does is set the road up for a lawsuit. If a suit is yuor intention, I'd rec'd hiring an attorney before you send the demand letter.

If it's a small change business, write letters, go to the media, picket in front of their place of business, etc etc.
